RULES AND REGULATIONS
Here are all the rules governing the purchase and, if you win, redemption of tickets for the drawing:
- The price is $40 per ticket or 3 tickets for $100, payable by check, cash, or credit card. Checks must be made payable to Cincinnati Children's Hospital Medical Center and mailed to: Cincinnati Children's Hospital Medical Center, Win A Wine Cellar IX, Division of Rheumatology MLC 4010, 3333 Burnet Avenue, Cincinnati, Ohio 45229.
- The tickets will be collected in the order payment is received. Checks that are denied or returned for insufficient funds or other reasons will disqualify the ticket covered by such check and the ticket will not be entered in the drawing. The same disqualification policy applies to invalid credit card payments.
- Prizes will be awarded in the order in which tickets are drawn, i.e., first ticket drawn wins first prize.
- All tickets must be purchased prior to May 12, 2008.
- The maximum number of tickets that will be sold is 900. Ticket sales will cease when that number is reached, and any checks received after May 15, 2008, will be returned to the sender. With 23 prizes offered, the odds of winning are approximately 1 in 39 assuming all 900 tickets are sold and subject to the conditions stated herein.
- Prizes will be scaled to the number of tickets sold. Minimum requirement for prizes as shown is the sale of 500 tickets. In the event fewer than 500 tickets are sold, prizes awarded will be scaled to a percentage of tickets sold versus the minimum goal of 500 tickets.
- All prizes are subject to, and the winners are fully responsible for federal, state and local income taxes.
- Winners must accept delivery of the prizes by June 15, 2008, or the prizes will be forfeited.
- There shall be no substitution for the listed prizes. No cash equivalent will be awarded.
- Net proceeds will benefit The Arthritis Clinic at Cincinnati Children's Hospital Medical Center and The Arthritis Foundation, Ohio River Valley Chapter.
- Winners need not be present to win.
- Winners will be notified by phone or email and their names will be posted on this web site no later than May 21, 2008.
- Ticket purchasers must be 21 years of age or older.
- When we receive your ticket order, we will mail you ticket stubs and a letter of thanks showing that you participated in this year's drawing.
- Your cancelled check is your receipt.
- Employees and members of immediate families of employees are eligible to win.
- Drawing to be held in the Levinson Library, Division of Rheumatology at Cincinnati Children's Hospital Medical Center at noon on Thursday, May 15, 2008.
